Group Health Insurance

Small Group (1-99 Eligible): If you are unsure whether or not you qualify as a small group, please see our FAQ or feel free to call us.

Auxilian represents all the major health insurers in California and we have clients in several other states as well, however if you are interested in a specific carrier or State that we do not currently serve please contact us and we can probably get the necessary authorizations very quickly

If you know what you need, you can download our quote request form:

Competed forms can be emailed to us (see contact page) or faxed to us at: (415) 858-0065

In most cases it will take us less than 1 business day to provide you with a formal quote. We look forward to working with you.

Large Group (100 Employees or More): Health Insurance for larger groups is regulated and sold differently than for Small Groups and is based on a Request for Quote (RFQ) process. We would be happy to work with you to put a cost-effective and valuable benefits package in place. If you are coming up on your open enrollment soon, are concerned about changes you might need to make under PPACA or just want to brainstorm with us to have a new set of eyes review to your benefits, please contact either Rick or David directly to discuss your needs and what we can do to help.

Individuals and Families: Please see our Individuals & Families page.

Dental Plans

Dental plans are a great benefit for your employees, encouraging good dental health, which is an important part of a comprehensive wellness program. In choosing a dental plan the key decisions are the amount of coverage (maximum annual benefit), the deductibles and copays for preventive, basic and major services, any difference between coverage with preferred (in-network) versus non-preferred providers and added features such as orthodontics or carry-overs.

Dental HMO's, which provide a benefit only with HMO-contracted dentists are less expensive, with prices for a single member starting at less than $20 per month depending on the specific design. PPO-based products, which combine a network based plan with an option to use non-network dentists are often more expensive and the price will vary from the mid-$20 range to over $70 per member per month again depending on the features.

To get a quote, you can use the Request for Quote forms above. It is useful but not necessary to coordinate your dental benefit with your health insurance however some insurers will only offer dental coverage to their health insurance customers and of course it is convenient for employers and employees to have the open enrollment period for all of their benefits at the same time. Additionally, some carriers will offer discounts when more than one product is purchased.

Vision Plans

Vision plans are a very popular benefit for employees and, like dental they can be purchased on a stand-alone basis or in conjunction with other benefits. Modest discounts may be available when it is bundled with other benefits.

Vision plans also have a relatively small set of key features to compare including the deductibles and copays (if any) for checkups, frames, lenses and contacts, the frequency of allowed services, and the maximum in and out of network benefits. The price will usually range between $8 and $20 per member with the least expensive plans allowing new frames and lenses every 24 months and the most expensive having low or no deductibles and copays and allowing new frames and lenses every 12 months. An average plan that has a $10-$15 copay, annual checkups, new lenses every 12 months and new frames every 24 months will often cost around $15 or less per month.

Again, you can use the Request for Quote forms above to contact us or callus if we can answer any questions you have.

Group Term Life Insurance

Group term life insurance is available on both a guaranteed-issue basis and an underwritten basis (employees must submit some form of evidence of insurability).  The amount of coverage available will depend on the insurer and the group size and can be based on either a fixed amount or a percentage of salary.

Guaranteed Issue amounts can be as low as $10,000 for the smallest groups, but for modest-sized  groups we can usually get over $100,000 or higher. Sometimes the offering will be a hybrid such as 1 times salary up to a maximum of $125,000 and the cost is usually only a few dollars per employee. This is normally a company-paid benefit and a great addition to your overall benefits program.

Disability Insurance

Disability insurance is probably one of the most neglected but important tools people can use to protect their financial situation because it protects employees if they are unable to work because of injury or illness. You can buy Short-Term Disability, which supplements State Disability payments and Long-Term Long-Term Disability, which starts once Short-Term benefits are exhausted.

Disability coverage is always based on salary and typically provides 60% of salary up to some maximum amount that can be chosen by the company. It can be a company-paid or employee-paid benefit and if paid for on an after-tax basis the benefits are non-taxable.

For companies with 15 or more employees, dental (and vision) plans can be done on a self-funded, or partially self-funded basis in which the employer uses a third party to administrator the plan, but pays the claims itself. Since the risks in these plans is limited and can be very well controlled by designing the plans correctly, this can be a very cost-effective approach.

Health Insurance can also be provided through self funding. Partial self-funding is a good option for most companies but true self-insurance, where the company retains most or all of the claims risk, is appropriate for companies with several hundred or more employees.

We are very experienced in all aspects of insurance and risk transfer so we can help you determine and implement a prudent and cost-effective strategy for your business.

Other Benefits

Health insurance, dental, vision and life insurance are the most commonly-offered benefits but many other products are available. Call or write us us for more information on a wide range of products which can make your overall benefits package more attractive, often at no additional cost to the company and sometimes no cost to the employees.
